Quillstone Course Portal 9.1 (fictional product): example Accessibility Conformance Report

Example report for a fictional product. 38 criteria reported at Levels A and AA (WCAG 2.0 rows): 23 Supports, 11 Partially Supports, 2 Does Not Support, 2 Not Applicable. The gaps that matter most are the missing focus outline in the student view, keyboard-inaccessible matching questions, missing audio description and the untagged PDF export reported under 504.2.2.

Example only. Quillstone Course Portal 9.1 (fictional product) is a made-up product, and every finding below is invented to show how a filled report reads. It is not a report on any real product.

Product
University course portal
Scope
Example report for an invented product; Quillstone Course Portal and its vendor do not exist. Covers version 9.1 of the hosted portal at demo.quillstone.example in the student and instructor roles: course home, modules, discussion forums, assignments and submission, quizzes, gradebook, calendar, the built-in lecture video player, the course page editor and the help centre. Course material uploaded by institutions and the third-party live conferencing integration are out of scope. Standards covered: WCAG 2.0 Levels A and AA, and the Revised Section 508 Standards (36 CFR 1194, as corrected January 22, 2018).
Evaluation methods
Manual testing of every in-scope screen and of the complete processes for enrolling, submitting an assignment, taking a quiz and building a course page, by a tester trained in the Trusted Tester process and an instructional designer who uses the portal. Keyboard only; JAWS and NVDA with Chrome and Firefox; VoiceOver with Safari; ZoomText; Dragon voice control. Automated checks with axe-core. Exported PDFs checked with a PDF/UA validator.
What it shows
A course portal is also an authoring tool, so section 504 applies and the PDF export becomes a Section 508 finding. The report does not claim the 501.1 exception for web applications, because that exception depends on full WCAG 2.0 A and AA conformance, which the portal does not yet have.

Table 1: Success Criteria, Level A
CriteriaConformance levelRemarks and explanations
1.1.1 Non-text ContentPartially SupportsInterface icons and buttons have text alternatives. Equations made with the math tool render as images whose alt text is the raw LaTeX source, which a screen reader reads as code. Authoring tool: the editor prompts for alt text when an image is inserted.
1.2.1 Audio-only and Video-only (Prerecorded)Not ApplicableThe portal itself contains no audio-only or video-only content. Media uploaded by instructors is outside this report.
1.2.2 Captions (Prerecorded)SupportsThe nine tutorial videos have edited captions, and the lecture player displays caption files that instructors attach.
1.2.3 Audio Description or Media Alternative (Prerecorded)Partially SupportsSix tutorial videos have transcripts. Three screen walkthroughs have neither a transcript nor audio description.
1.3.1 Info and RelationshipsPartially SupportsThe gradebook is a data table with row and column headers, and forum threads use nested lists. Quiz answer options are not grouped with their question, so a screen reader announces the options without it.
1.3.2 Meaningful SequenceSupportsReading order matches the visual order.
1.3.3 Sensory CharacteristicsSupportsInstructions do not rely on shape, size, position or sound.
1.4.1 Use of ColorPartially SupportsLate submissions in the gradebook are marked only by an orange cell background.
1.4.2 Audio ControlSupportsNo audio plays automatically. Answered Supports rather than Not Applicable, following the template's note on criteria with no applicable content.
2.1.1 KeyboardPartially SupportsDrag-and-drop matching questions cannot be answered with a keyboard. All other question types and all other functions can.
2.1.2 No Keyboard TrapSupportsIn the course page editor Tab indents; Escape followed by Tab leaves the editor, and the help centre documents this.
2.2.1 Timing AdjustablePartially SupportsInstructors can set quiz time limits and grant extra time to named students, but a student cannot adjust or extend a limit, and no essential-time exception is claimed.
2.2.2 Pause, Stop, HideSupportsThe announcement banner that scrolls has a pause button.
2.3.1 Three Flashes or Below ThresholdSupportsNo content flashes.
2.4.1 Bypass BlocksSupportsSkip links lead to the course navigation and to the main content.
2.4.2 Page TitledPartially SupportsPages inside a course module all share the title "Course content | Quillstone" and do not name the module or page.
2.4.3 Focus OrderSupportsFocus order is logical, and dialogs move focus in and back out.
2.4.4 Link Purpose (In Context)Partially SupportsThe assignment list repeats "Submit" links that do not include the assignment name.
3.1.1 Language of PageSupportsEvery page declares its language.
3.2.1 On FocusSupportsNo change of context on focus.
3.2.2 On InputSupportsSelecting a course in the course switcher does nothing until Go is pressed.
3.3.1 Error IdentificationSupportsErrors are identified in text beside the field.
3.3.2 Labels or InstructionsSupportsFields have visible labels, and file upload states the accepted formats and size.
4.1.1 ParsingSupportsFor WCAG 2.0 and the Revised 508 Standards, the September 2023 errata indicates this criterion is always supported.
4.1.2 Name, Role, ValuePartially SupportsThe course navigation tree does not expose the expanded or collapsed state of modules. Editor toolbar buttons have names and states.
